At TDHS we offer many services and over the coming months we are going to showcase our services in a series called
“A Spotlight on our Services”
Today we are highlighting the Hospital Admission Risk Program (HARP)
HARP is a program assisting people to maintain independence and improve their quality of life.
Kate, HARP Care Coordinator, is an RN who can assist people in the Timboon district area. Kate is located onsite at TDHS every Tuesday and Wednesday.
She can assist people to better understand their chronic health conditions and help them to navigate the health system, whilst working closely with their GP and other health professionals.
Kate will help you set health goals and put relevant assistance in place to help you achieve these. The ultimate goal of HARP is for the individual to be able to self-manage and remain safely in their own home.
Face to face consultations are available at TDHS or at home. Alternatively, telehealth consults can be conducted. Referrals can be made by GPs, Health Professionals or self.
